Anyway, we got everything from www.putting-greens.com because my uncle who is on the PGA Tour has 3 in his yard and he got everything from there.
you order everything you need... you can get the kit but, we ended up needing more than what was in the kit so, we aren't going to get the kit when we do it at our other house.
you need:
the manual
easy cup cutter
practice flag/cup
the bentgrass seed. depending on where you live... you might want to get one in the bucket... if its hot/humid where you live.
there is different kinds of everything... the cutter and the flags. it doesn't matter which one you get.. depends on how much you want to spend etc.
once you get everything.. i would suggest starting on early friday or saturday morning. once everything is done and in place, you will have a putting green!!! make sure you water it in hot weather and that simple stuff. they are easy to maintain and you can put rocks around them to look pretty.. its all about what you want.

They had a wide selection of products and step-by-step installation. I would recommend their champion putting green as it looks like it is the best and they said it runs from 10-12 on the stimpmeter which is good for practicing on.
Also while looking around they have fringe material, which would be good for setting up a spot to ship from too. You could get a thick durable plastic tarp and with digging hole and creative landscaping you could add a bunker and mke it a complete backyard shortgame paradise area.

Go with artificial turf. The knowledge to maintain a disease free green is best left to the experts. Unless you have a good Greenskeeper you will go nuts trying to keep the green in shape. Also the mowers needed to mow a real green are tempermental and costly.
